Job Title: Eldercare Navigator – Licensed Clinician (RN, LCSW, NP, or PA)

Job Type: Part-Time | Remote | $95+ / hour depending on licensure

Location: Remote (U.S.-based)

OneVillage is a leading virtual care platform providing whole-person support to individuals and families navigating serious health challenges, including aging, caregiving, and cognitive decline. We’re hiring a licensed clinician to serve as an Eldercare Navigator—delivering virtual care planning, education, and support to older adults and their caregivers through reimbursable services.

This role is ideal for a seasoned RN, LCSW, NP, or PA who brings deep empathy and experience supporting aging populations. You’ll help us deliver best-in-class care while also supporting our transition into insurance reimbursement pathways.

• Deliver virtual eldercare and cognitive health support services eligible for reimbursement under relevant CPT codes, including:
• 99483 – Care planning for cognitive impairment
• 99401–99404 – Preventive counseling
• 98960–98962 – Education for self-management of chronic conditions
• Conduct structured assessments of seniors and caregivers
• Create and document customized care plans, including clinical notes and ICD-10 coding
• Educate families about disease progression, home safety, long-term care, and support resources
• Collaborate with our broader care team of coaches, navigators, and clinical advisors
• Assist with credentialing and payer enrollment activities as we expand reimbursable services

• Active U.S. license as a RN, LCSW, NP, PA, or other qualified clinician
• Minimum 3+ years experience supporting older adults, caregivers, or individuals with dementia, Alzheimer’s, or complex chronic conditions
• Familiarity with CPT codes for preventive counseling and cognitive care planning
• Strong documentation skills, including ICD-10 coding and virtual care charting
• Experience delivering trauma-informed, culturally responsive care

• Multistate licensure or willingness to expand to additional states
• Knowledge of Medicare and commercial insurance reimbursement workflows
• Experience with cognitive screenings (e.g., MoCA, ADLs/IADLs assessments)
• Comfort supporting diverse, multicultural families navigating aging challenges
