We are currently hiring for a seasoned licensed clinician to serve as an Eldercare Navigator, delivering virtual care planning, education, and support to older adults and their caregivers through reimbursable services. This role is ideal for an LCSW, NP, or PA with deep clinical expertise, exceptional empathy, and a drive to lead change in the aging and caregiving space.

Key Responsibilities
  • Deliver virtual eldercare, aging-in-place-support and cognitive health support services eligible for reimbursement under relevant CPT codes, including:

99483 – Care planning for cognitive impairment

99401–99404 – Preventive counseling

98960–98962 – Education for self-management of chronic conditions

  • Conduct structured assessments of seniors and caregivers
  • Create and document customized care plans, including clinical notes and ICD-10 coding
  • Educate families on disease progression, home safety, LTC, and available resources
  • Collaborate with our care team of coaches, navigators, and clinical advisors
  • Support credentialing and payer enrollment activities
Qualifications

Required:

● Active U.S. license as LCSW, NP, or PA

● Preference for licensure in Virginia, Maryland, or Washington, D.C., with willingness to obtain additional state licenses

● Minimum 3+ years of experience supporting older adults, caregivers, or individuals with dementia, Alzheimer’s, or complex chronic conditions

● Familiarity with CPT codes related to preventive counseling and cognitive care planning

● Strong documentation skills, including ICD-10 coding and virtual care charting

● Ability to deliver trauma-informed, culturally responsive care

Preferred:

● Multistate licensure or willingness to obtain additional licenses

● Knowledge of Medicare and commercial insurance reimbursement workflows

● Experience with cognitive assessments (e.g., MoCA, ADLs/IADLs)

● Comfort supporting diverse, multicultural families through aging challenges

● Prior experience in related settings such as senior living communities, CCRCs, home health, or aging services organizations

● Strong interpersonal skills with ability to effectively communicate to multiple generations of family members

● Comfort with and interest in technology solutions to help caregivers and their families
