| Position Title: Licensed Practical Nurse (LPN) Home Coordinator LOCATION: Middle Tennessee - Two IDD Medical Homes STATUS: Full Time, Salaried, Exempt PROGRAM: Developmental Disabilities Services REPORTS TO: Director of Community INTRODUCTION: Volunteers of America Mid-States (VOA) is a non-profit organization spanning four states that creates positive change in the lives of individuals and communities through a ministry of service. This position ensures individuals with intellectual and developmental disabilities receive safe, person-centered, medically appropriate, and high-quality supports in homes that promote dignity, choice, independence, community inclusion, and quality of life. The LPN Home Coordinator manages daily home operations, supervises LPN and Direct Support Professional (DSP) staff, coordinates healthcare services, monitors medication and health documentation systems, supports appointment follow-through, and ensures compliance with applicable Tennessee DIDD, Medicaid, HCBS, licensing, and agency requirements. The position practices within the authorized LPN scope of practice and seeks appropriate clinical direction when matters require a Registered Nurse, physician, advanced practice provider, or other qualified clinician. WHAT YOU SHOULD HAVE FOR THIS ROLE:
